Heart failure is not simply “a weak heart.” It is a clinical syndrome in which the heart cannot maintain normal circulation without elevated filling pressures, reduced output, or both. The nursing job is to recognise whether the patient is congested, poorly perfused, deteriorating, or stable — and act on what matters first.
